What is a Heil- und Kostenplan?
Before you receive dentures, crowns or bridges in Germany, your dentist draws up a Heil- und Kostenplan (HKP), a treatment and cost plan. It describes your findings, the standard care defined by statutory health insurance and – if you choose something else – the treatment actually planned, with estimated costs. Your insurer sets your fixed subsidies on this basis. Treatment should only start after approval.
How the plan is structured
- I. Findings and treatment plan: the tooth chart with three rows. B (Befund) shows the condition of each tooth, R (Regelversorgung) the standard care, TP (Therapieplanung) the treatment actually planned. The TP row is only filled in if you choose a different or upgraded treatment.
- II. Findings for fixed subsidies: finding number (e.g. 1.1), tooth or area, quantity and amount.
- III. Cost planning: fees under BEMA (statutory), fees under GOZ (private, estimated), material and lab costs (estimated) and total treatment costs.
- IV. Subsidy level: 60, 70 or 75 % depending on the bonus booklet, and the “Härtefall” (hardship) field.
Finding numbers and fixed subsidies
Each finding number stands for a situation defined in the fixed-subsidy guideline, e.g. 1.1 for a preservable tooth with extensive destruction. Each number has a fixed nationwide amount: 60 % of standard care, 70 % with 5 years and 75 % with 10 years of an unbroken bonus booklet. Standard care, same type with extras, different type
Standard care is the reference treatment for your finding. If you choose the same type with extras (e.g. all-ceramic instead of metal), it is gleichartig: your subsidy stays the same and you pay the extra costs. If you choose a different type (e.g. implant crown instead of a bridge), it is andersartig: you receive an invoice for the total costs and your insurer reimburses the approved subsidy.
Frequently asked questions
What do the abbreviations in the tooth chart mean?
They come from a binding list of the National Association of Statutory Health Insurance Dentists (KZBV). Lower-case letters describe the finding (e.g. f = missing tooth, ww = preservable tooth with extensive destruction, kw = crown in need of renewal), capitals the treatment (e.g. K = crown, B = pontic, KM = all-ceramic or fully veneered crown). How can I tell whether my subsidy is correct?
Each finding number has a fixed amount per bonus level. The explainer compares the amounts in your plan with the official 2026 values and recalculates the totals. Differences are flagged – they may be reading errors or have a reason your dentist can explain.
Why is my share so high?
The fixed subsidy covers only a set share of standard care. If you choose more, such as all-ceramic or implants, you pay the extra costs. Ask about the cost of standard care only to compare.
Is my data stored?
No. Your files are processed in memory on our server and sent to OpenAI for text recognition. Only finding numbers, abbreviations, amounts and percentages come back. Ideally cover your name and insurance number before uploading.
How accurate is the analysis?
Text recognition reads your document automatically and can make mistakes, especially with blurry photos. All explanations, checks and calculations afterwards are based on the official subsidy amounts and the KZBV abbreviation list. Always compare the values read with your plan.
Can I check a plan from an earlier year?
We explain abbreviations and treatment types for any plan since July 2022. Amounts are only checked against the 2026 subsidies, because the amounts change every year.
Does the tool replace advice from my dentist?
No. The explainer helps you understand your plan and ask the right questions. Which treatment suits you is something to discuss with your dentist. We are also happy to offer a second opinion.
