Even those who brush carefully twice a day and use dental floss regularly cannot adequately clean all areas of the mouth. In interdental spaces, along the gum line and on the backs of teeth, an invisible, sticky bacterial film — known as biofilm, commonly called plaque — forms within just a few hours.

This biofilm is the root cause of almost all dental diseases: gum inflammation (gingivitis), periodontitis, cavities and even bad breath develop when biofilm remains on tooth surfaces for too long. When biofilm calcifies through mineral salts from saliva, it becomes tartar — a hard, chalky deposit that can no longer be removed with a toothbrush and can only be eliminated using professional instruments.

In addition, external staining from coffee, tea, red wine or nicotine becomes embedded in the tooth surface and dulls your smile. These discolourations can also only be effectively and gently removed by professional cleaning techniques — particularly through the Air-Flow technique.

1. Assessment and Oral Hygiene Status

Every PZR begins with a brief examination: we check the condition of your teeth, gums and oral hygiene. We measure gum pocket depths at selected sites and document any signs of inflammation. This step takes only a few minutes but provides important guidance on which areas need special attention and which cleaning method is most suitable for you.

2. Plaque Staining — Making Biofilm Visible

A special staining solution (plaque disclosing agent) is used to colour all biofilm deposits — red and blue areas show you and us precisely where cleaning is needed. This is not only valuable for the treatment itself, but also an important learning opportunity for you: you immediately see which spots your toothbrush regularly misses at home. This visual feedback moment is a unique element of the PZR.

3. Ultrasonic Tartar Removal

Tartar — the calcified form of biofilm — can only be removed with professional instruments. Our modern ultrasonic device generates fine sound waves that gently and effectively break down the tartar. At the same time, the working area is rinsed with water, making the treatment more comfortable and flushing away bacteria. The ultrasound also reaches areas that are difficult to access with manual instruments — particularly at the gum line and in shallow gum pockets.

4. Air-Flow / Powder Jet Technology

After tartar removal, stubborn discolourations and the finest biofilm residues are addressed. The Air-Flow procedure (EMS powder jet technology) applies a fine mixture of water, air and specially developed powder with gentle pressure to the tooth surfaces. The result: coffee stains, tea stains and nicotine deposits virtually disappear without a trace. The procedure is gentler than many patients expect — modern powders (e.g. glycine powder) are so fine that they do not damage tooth enamel and can even be used on sensitive gum tissue.

5. Polishing

After cleaning, all tooth surfaces are polished with a special polishing paste and a rotating brush or rubber polisher. Polishing smooths fine irregularities and micro-fissures in the enamel that would otherwise serve as attachment points for new biofilm. A smooth tooth surface is noticeably slicker to the tongue test — bacteria find less to hold on to, and new deposits form more slowly.

6. Fluoride Application

Every PZR concludes with fluoride application: a high-concentration fluoride varnish is applied to all tooth surfaces and left to act for several minutes. Fluoride hardens the enamel, making it more resistant to acid attacks and thus actively preventing cavity formation. This protective film is particularly valuable after mechanical cleaning, as the tooth surface is freshly cleaned and especially receptive.

With age, the gums often recede and expose tooth roots. Root cementum is softer than enamel and therefore considerably more susceptible to decay. The PZR therefore plays a particularly important role for seniors: it removes deposits from exposed root surfaces and protects through intensive fluoride application. Orthodontic Patients with Fixed Braces

Fixed orthodontic appliances — brackets and wires — make home oral hygiene considerably more difficult. Biofilm and carious lesions form preferentially around brackets. For orthodontic patients we recommend PZR appointments every two to three months. PZR and General Health — More than Just Clean Teeth

The connection between oral health and general health is well documented scientifically — and is still underestimated by many patients. The mouth is not an isolated organ: bacteria and inflammatory mediators from the oral cavity can influence the entire body via the bloodstream.

Cardiovascular disease: Patients with chronic periodontitis have a significantly elevated risk of heart attack and stroke. Periodontal bacteria (particularly Aggregatibacter actinomycetemcomitans and Porphyromonas gingivalis) have been detected directly in atherosclerotic plaques. Regular PZR lowers the total bacterial count and thereby reduces systemic inflammatory markers such as CRP.

Swedish dentist Per Axelsson demonstrated impressively in a 30-year longitudinal study: consistent professional prophylaxis reduces cavity formation by up to 70% and virtually completely prevents periodontitis-related tooth loss. These data are still considered a milestone in preventive dentistry today.

Why do prices vary? Before & After — What to Expect Following Your PZR

Many patients are surprised by how noticeable the difference is after a professional dental cleaning — both visually and in terms of how it feels. Here we describe realistically what you will experience immediately after treatment and in the following weeks.

Immediately after the PZR: Your teeth feel smooth and fresh — a feeling you only truly appreciate once you have experienced it. Staining from coffee, tea or red wine is significantly reduced or completely gone. The gums may remain slightly sensitive for a short time, which is completely normal and resolves within 24 to 48 hours. The fluoride application leaves a slightly chalky film on the teeth — this is intentional and shows that the fluoride varnish is working.

In the first few days: The gums, which were previously slightly swollen due to inflammation, recede and appear more defined. In some patients, the teeth appear slightly lighter after the PZR — this is not due to bleaching, but because the natural tooth colour comes back into view once discolourations have been removed. Long-term: With regular PZR, the gums remain healthier, cavities occur less frequently, and tartar builds up more slowly. Patients who consistently attend for prophylaxis have measurably shallower gum pockets and significantly fewer tooth losses with age. Most patients find the PZR pleasant or at most slightly uncomfortable. Pain in the true sense rarely occurs. With inflamed gums or exposed tooth necks, brief sensitivity can arise — we adapt the treatment intensity at all times. On request, we can apply a numbing gel before treatment. After the PZR, teeth can be slightly sensitive for 1–2 days, which is completely normal and temporary.
The duration depends on the method chosen and the degree of deposits. A standard PZR takes approx. 45–60 minutes. The Air-Flow PZR likewise 45–60 minutes, GBT dental cleaning 60–75 minutes. With heavy tartar deposits or implants it may take a little longer. For children, treatment is generally shorter (30–40 min.).
Air-Flow is a technique (EMS powder jet technology) in which a fine water-air-powder mixture removes staining and biofilm. GBT (Guided Biofilm Therapy) is a complete treatment protocol built on Air-Flow: first the biofilm is stained, then systematically removed with Air-Flow, followed by ultrasound cleaning only where necessary, and finally polishing and fluoride application. GBT is the gentlest and most modern protocol — particularly suitable for patients with implants or sensitive teeth.
After the PZR we recommend not eating or drinking for at least 30 minutes — so the fluoride varnish can take full effect. During the first two hours you should avoid strongly staining foods such as coffee, tea, red wine or turmeric, so that the freshly cleaned tooth surfaces do not take on new staining. After that you can eat and drink normally.
Yes, the PZR is not only safe during pregnancy, but expressly recommended. Hormonally induced pregnancy gingivitis is common and can lead to periodontitis if untreated, which is in turn associated with increased risk of premature birth. The ideal time is the second trimester (14th–27th week of pregnancy). X-rays are of course avoided. Please inform us at the start of your appointment about your pregnancy.
As soon as the first permanent teeth emerge — generally from around age 6 — we recommend the first children's PZR. The treatment includes staining and removal of deposits, fluoride application and brushing training. Children at high caries risk (sugar-rich diet, cleft lip/palate, disabilities) should come more frequently. Yes, in most cases significantly. The most common cause of bad breath is bacterial deposits on the tongue, gums and between the teeth, as well as gum inflammation. The PZR systematically removes these deposits and simultaneously addresses the underlying inflammation. We also advise you on tongue cleaning and special oral care products that reduce halitosis in the long term.
The PZR removes external staining (from coffee, tea, red wine, nicotine) and brings the natural tooth colour back into view. This can certainly feel like a slight brightening. Actual bleaching — i.e. chemical lightening of the inner tooth colour — is a separate treatment. If you wish for a noticeably lighter tooth shade, we recommend our professional bleaching after the PZR as the optimal combination.
Plaque (also called biofilm) is a soft, sticky bacterial film that forms daily on tooth surfaces and can be removed by thorough brushing and flossing. If plaque is not removed, it mineralises through calcium and phosphate salts from saliva and becomes hard tartar. Tartar can no longer be removed by home oral hygiene — it can only be eliminated using professional instruments (ultrasound, hand instruments).
New plaque begins to form within just a few hours — this is completely normal. Tartar builds up to significant amounts in most people after about three to six months. How quickly deposits form varies individually and depends on saliva composition, diet and home oral hygiene. With consistent daily care you significantly extend the period until the next PZR.
Yes, implant PZR requires special instruments and particular care. Metallic hand instruments would damage the titanium surface of the implant and abutment — we therefore use carbon and plastic curettes. Ultrasound tips must be specifically selected for implants. For implant PZR, shorter intervals are recommended (every three to four months), as peri-implantitis (inflammation around the implant) progresses more rapidly than natural periodontitis.
Without regular PZR, tartar builds up over the years — particularly below the gum line. This promotes chronic gum inflammation (gingivitis) that can develop into periodontitis without treatment: a gradual bone loss that ultimately leads to tooth loss. At the same time, the risk of cavities increases as biofilm produces acids unimpeded. The good news: it is never too late to start — even after years of neglect, we can help.

At the same time, the working area is rinsed with water, making the treatment more comfortable and flushing away bacteria. The ultrasound also reaches areas that are difficult to access with manual instruments — particularly at the gum line and in shallow gum pockets.

4. Air-Flow / Powder Jet Technology

After tartar removal, stubborn discolourations and the finest biofilm residues are addressed. The Air-Flow procedure (EMS powder jet technology) applies a fine mixture of water, air and specially developed powder with gentle pressure to the tooth surfaces. The result: coffee stains, tea stains and nicotine deposits virtually disappear without a trace. The procedure is gentler than many patients expect — modern powders (e.g. glycine powder) are so fine that they do not damage tooth enamel and can even be used on sensitive gum tissue.

5. Polishing

After cleaning, all tooth surfaces are polished with a special polishing paste and a rotating brush or rubber polisher. Polishing smooths fine irregularities and micro-fissures in the enamel that would otherwise serve as attachment points for new biofilm. A smooth tooth surface is noticeably slicker to the tongue test — bacteria find less to hold on to, and new deposits form more slowly.

6. Fluoride Application

Every PZR concludes with fluoride application: a high-concentration fluoride varnish is applied to all tooth surfaces and left to act for several minutes. Fluoride hardens the enamel, making it more resistant to acid attacks and thus actively preventing cavity formation. This protective film is particularly valuable after mechanical cleaning, as the tooth surface is freshly cleaned and especially receptive.

With age, the gums often recede and expose tooth roots. Root cementum is softer than enamel and therefore considerably more susceptible to decay. The PZR therefore plays a particularly important role for seniors: it removes deposits from exposed root surfaces and protects through intensive fluoride application. For patients with limited fine motor skills, we are happy to take over a larger portion of the daily cleaning.

Orthodontic Patients with Fixed Braces

Fixed orthodontic appliances — brackets and wires — make home oral hygiene considerably more difficult. Biofilm and carious lesions form preferentially around brackets. For orthodontic patients we recommend PZR appointments every two to three months. PZR and General Health — More than Just Clean Teeth

The connection between oral health and general health is well documented scientifically — and is still underestimated by many patients. The mouth is not an isolated organ: bacteria and inflammatory mediators from the oral cavity can influence the entire body via the bloodstream.

Cardiovascular disease: Patients with chronic periodontitis have a significantly elevated risk of heart attack and stroke. Periodontal bacteria (particularly Aggregatibacter actinomycetemcomitans and Porphyromonas gingivalis) have been detected directly in atherosclerotic plaques. Regular PZR lowers the total bacterial count and thereby reduces systemic inflammatory markers such as CRP.

Diabetes mellitus: There is a bidirectional relationship between periodontitis and diabetes: poorly controlled blood sugar promotes gum inflammation, and conversely, untreated periodontitis worsens blood sugar control. Swedish dentist Per Axelsson demonstrated impressively in a 30-year longitudinal study: consistent professional prophylaxis reduces cavity formation by up to 70% and virtually completely prevents periodontitis-related tooth loss. These data are still considered a milestone in preventive dentistry today.

Why do prices vary? The decisive factors are: the number of teeth to be cleaned, the degree of deposits (little or a lot of tartar?), the method used (standard PZR, Air-Flow or GBT), and whether implants need to be cleaned. What do statutory health insurers pay?

Statutory health insurance does not cover the PZR as a standard benefit — it is an individual health service (IGeL). Immediately after the PZR: Your teeth feel smooth and fresh — a feeling you only truly appreciate once you have experienced it. The gums may remain slightly sensitive for a short time, which is completely normal and resolves within 24 to 48 hours. Can you eat and drink immediately after a dental cleaning?
After the PZR we recommend not eating or drinking for at least 30 minutes — so the fluoride varnish can take full effect. During the first two hours you should avoid strongly staining foods such as coffee, tea, red wine or turmeric, so that the freshly cleaned tooth surfaces do not take on new staining. After that you can eat and drink normally.
Is a professional dental cleaning safe during pregnancy?
Yes, the PZR is not only safe during pregnancy, but expressly recommended. Hormonally induced pregnancy gingivitis is common and can lead to periodontitis if untreated, which is in turn associated with increased risk of premature birth. We also advise you on tongue cleaning and special oral care products that reduce halitosis in the long term.
Can a professional dental cleaning whiten teeth?
The PZR removes external staining (from coffee, tea, red wine, nicotine) and brings the natural tooth colour back into view. This can certainly feel like a slight brightening. Actual bleaching — i.e. chemical lightening of the inner tooth colour — is a separate treatment. What is the difference between tartar and plaque (biofilm)?
Plaque (also called biofilm) is a soft, sticky bacterial film that forms daily on tooth surfaces and can be removed by thorough brushing and flossing. If plaque is not removed, it mineralises through calcium and phosphate salts from saliva and becomes hard tartar. Tartar can no longer be removed by home oral hygiene — it can only be eliminated using professional instruments (ultrasound, hand instruments).
How long do teeth stay clean after a PZR?
New plaque begins to form within just a few hours — this is completely normal. Tartar builds up to significant amounts in most people after about three to six months. How quickly deposits form varies individually and depends on saliva composition, diet and home oral hygiene. With consistent daily care you significantly extend the period until the next PZR.
Is the PZR for dental implants different from that for natural teeth?
Yes, implant PZR requires special instruments and particular care. Metallic hand instruments would damage the titanium surface of the implant and abutment — we therefore use carbon and plastic curettes. Ultrasound tips must be specifically selected for implants. For implant PZR, shorter intervals are recommended (every three to four months), as peri-implantitis (inflammation around the implant) progresses more rapidly than natural periodontitis.
What happens if you never have a professional dental cleaning?
Without regular PZR, tartar builds up over the years — particularly below the gum line. This promotes chronic gum inflammation (gingivitis) that can develop into periodontitis without treatment: a gradual bone loss that ultimately leads to tooth loss.
