What is a Professional Dental Cleaning?
A Professional Dental Cleaning — known as PZR in German — is a thorough, professionally performed cleaning of all tooth surfaces, interdental spaces and the gum line. It goes far beyond what your daily oral hygiene routine can achieve. What began as a general preventive measure has today become an indispensable part of modern dentistry.
Even those who brush carefully twice a day and use dental floss regularly cannot adequately clean all areas of the mouth. In interdental spaces, along the gum line and on the backs of teeth, an invisible, sticky bacterial film — known as biofilm, commonly called plaque — forms within just a few hours.
This biofilm is the root cause of almost all dental diseases: gum inflammation (gingivitis), periodontitis, cavities and even bad breath develop when biofilm remains on tooth surfaces for too long. When biofilm calcifies through mineral salts from saliva, it becomes tartar — a hard, chalky deposit that can no longer be removed with a toothbrush and can only be eliminated using professional instruments.
In addition, external staining from coffee, tea, red wine or nicotine becomes embedded in the tooth surface and dulls your smile. These discolourations can also only be effectively and gently removed by professional cleaning techniques — particularly through the Air-Flow technique.
A professional teeth cleaning in Munich at our practice in Oberföhring takes 45 to 75 minutes depending on the method chosen. During this time our trained dental hygienists systematically remove all deposits, polish the tooth surfaces and seal them with fluoride varnish — the result: a fresh, clean feeling that you carry home with you.
The PZR is recommended at least once a year; for higher-risk patients (smokers, diabetics, periodontitis patients, implant wearers) every three to six months. Many statutory health insurance providers contribute to the cost of a PZR — please check with your insurer for the exact subsidy.

Step-by-Step: The Professional Dental Cleaning Process
A professional dental cleaning follows a fixed, systematic protocol. This ensures that no area is missed and the treatment is as comfortable as possible for you. Here is exactly what to expect during your appointment with us:

1. Assessment and Oral Hygiene Status
Every PZR begins with a brief examination: we check the condition of your teeth, gums and oral hygiene. We measure gum pocket depths at selected sites and document any signs of inflammation. This step takes only a few minutes but provides important guidance on which areas need special attention and which cleaning method is most suitable for you.
2. Plaque Staining — Making Biofilm Visible
A special staining solution (plaque disclosing agent) is used to colour all biofilm deposits — red and blue areas show you and us precisely where cleaning is needed. This is not only valuable for the treatment itself, but also an important learning opportunity for you: you immediately see which spots your toothbrush regularly misses at home. This visual feedback moment is a unique element of the PZR.
3. Ultrasonic Tartar Removal
Tartar — the calcified form of biofilm — can only be removed with professional instruments. Our modern ultrasonic device generates fine sound waves that gently and effectively break down the tartar. At the same time, the working area is rinsed with water, making the treatment more comfortable and flushing away bacteria. The ultrasound also reaches areas that are difficult to access with manual instruments — particularly at the gum line and in shallow gum pockets.
4. Air-Flow / Powder Jet Technology
After tartar removal, stubborn discolourations and the finest biofilm residues are addressed. The Air-Flow procedure (EMS powder jet technology) applies a fine mixture of water, air and specially developed powder with gentle pressure to the tooth surfaces. The result: coffee stains, tea stains and nicotine deposits virtually disappear without a trace. The procedure is gentler than many patients expect — modern powders (e.g. glycine powder) are so fine that they do not damage tooth enamel and can even be used on sensitive gum tissue.
5. Polishing
After cleaning, all tooth surfaces are polished with a special polishing paste and a rotating brush or rubber polisher. Polishing smooths fine irregularities and micro-fissures in the enamel that would otherwise serve as attachment points for new biofilm. A smooth tooth surface is noticeably slicker to the tongue test — bacteria find less to hold on to, and new deposits form more slowly.
6. Fluoride Application
Every PZR concludes with fluoride application: a high-concentration fluoride varnish is applied to all tooth surfaces and left to act for several minutes. Fluoride hardens the enamel, making it more resistant to acid attacks and thus actively preventing cavity formation. This protective film is particularly valuable after mechanical cleaning, as the tooth surface is freshly cleaned and especially receptive.
7. Individual Oral Hygiene Counselling
The session closes with a personal consultation: using the staining findings, we show you where your weak spots are and give you concrete, practical tips — which toothbrush suits you, which dental floss or interdental brush is appropriate for your interdental spaces, and how to reliably control your biofilm at home. This individual counselling is the key to ensuring that the positive effects of the PZR last as long as possible.
Airflow, GBT or Ultrasound — Your Comparison
Modern dental practices today offer a variety of cleaning methods — and not every technique is equally suited to every patient. We explain the differences so you understand why we choose the optimal method for your individual situation.
| Ultrasound | Air-Flow (EMS) | GBT (Guided Biofilm Therapy) | |
|---|---|---|---|
| Method | Sound waves break down tartar | Powder-water jet removes deposits and stains | Biofilm-guided protocol: staining, Air-Flow, ultrasound, polishing |
| Comfort | Medium | High | Very high |
| Treatment duration | 45–60 min. | 45–60 min. | 60–75 min. |
| Ideal for | Heavy tartar, subgingival cleaning | Heavy staining, smokers, coffee/tea drinkers | Implants, sensitive teeth, periodontitis patients |
| Gentle on enamel | Yes (when correctly applied) | Very gentle (glycine powder) | Maximally gentle |
| Suitable for implants | With limitations (special tips required) | Yes | Yes (optimised for titanium and ceramic surfaces) |
Guided Biofilm Therapy (GBT) is currently the most modern protocol for professional dental cleaning. Developed by EMS (Switzerland), it is based on the principle of first staining all biofilm, then systematically removing it with Air-Flow, and finally using ultrasound only where hard tartar actually remains. The result: maximum preservation of tooth structure and gums with optimal cleaning depth.
At our practice in Oberföhring we use all three methods — depending on the initial situation, sensitivity and your personal preference. During the initial consultation we discuss together which method suits you best. No competitor in Bogenhausen or Oberföhring offers you this choice with the same level of personal advice.

Who Particularly Benefits from Professional Dental Cleaning?
In principle, everyone benefits from regular professional dental cleaning. However, there are certain groups of people for whom a PZR is not only advisable, but medically particularly significant:

Children and Adolescents (from approx. age 6)
As soon as the first permanent teeth emerge, the first professional dental cleaning is worthwhile. Children's teeth have a still porous enamel structure that is more susceptible to cavities. The PZR for children includes not only the actual cleaning but also playful brushing training and fluoride application — both of which are proven to be preventive. Our dental hygienists are experienced with children and create a relaxed atmosphere that lays the foundation for a positive dental experience. For more information, please visit our children's dentistry page.
Adults — Prophylaxis as an Investment
For adults we recommend the PZR at least once a year; twice for those with elevated caries or periodontitis risk. Regular attendance pays off in the long run: a Swedish study showed that patients with consistent prophylaxis over 30 years experienced only minimal tooth loss — compared to a control group without preventive care. The PZR is therefore not just about aesthetics, but solid preventive healthcare.
Pregnant Women — Protection for Mother and Child
During pregnancy, hormonal changes are substantial — the gums react more sensitively to bacteria and tend towards pregnancy gingivitis, a hormonally intensified gum inflammation. Studies document a link between untreated periodontitis and increased risk of premature birth. A PZR during pregnancy is safe, gentle and expressly recommended — ideally in the second trimester. Please speak to us; we are happy to advise you.
Implant Wearers — Protection against Peri-implantitis
Dental implants require a different type of cleaning than natural teeth: titanium or ceramic material must not be damaged by metallic instruments. At the same time, the tissue surrounding the implant is more susceptible to inflammation (peri-implantitis), which in the worst case can lead to implant failure. For implant PZR we use special carbon and plastic instruments as well as Air-Flow with glycine powder. Treatment every three to four months is recommended. Find out more at Implantology.
Seniors — Protection against Root Cavities
With age, the gums often recede and expose tooth roots. Root cementum is softer than enamel and therefore considerably more susceptible to decay. The PZR therefore plays a particularly important role for seniors: it removes deposits from exposed root surfaces and protects through intensive fluoride application. For patients with limited fine motor skills, we are happy to take over a larger portion of the daily cleaning.
Orthodontic Patients with Fixed Braces
Fixed orthodontic appliances — brackets and wires — make home oral hygiene considerably more difficult. Biofilm and carious lesions form preferentially around brackets. For orthodontic patients we recommend PZR appointments every two to three months. We work closely with orthodontic colleagues and coordinate the cleaning schedule with the respective treatment phase.
PZR and General Health — More than Just Clean Teeth
The connection between oral health and general health is well documented scientifically — and is still underestimated by many patients. The mouth is not an isolated organ: bacteria and inflammatory mediators from the oral cavity can influence the entire body via the bloodstream.
Cardiovascular disease: Patients with chronic periodontitis have a significantly elevated risk of heart attack and stroke. Periodontal bacteria (particularly Aggregatibacter actinomycetemcomitans and Porphyromonas gingivalis) have been detected directly in atherosclerotic plaques. Regular PZR lowers the total bacterial count and thereby reduces systemic inflammatory markers such as CRP.
Diabetes mellitus: There is a bidirectional relationship between periodontitis and diabetes: poorly controlled blood sugar promotes gum inflammation, and conversely, untreated periodontitis worsens blood sugar control. Studies show that periodontitis treatment in diabetic patients can lower HbA1c values by up to 0.4%.
Pregnancy complications: As described in the section on pregnant women, treated periodontitis is associated with an increased risk of premature birth. Preventive PZR before and during pregnancy is therefore part of modern preventive medicine.
Swedish dentist Per Axelsson demonstrated impressively in a 30-year longitudinal study: consistent professional prophylaxis reduces cavity formation by up to 70% and virtually completely prevents periodontitis-related tooth loss. These data are still considered a milestone in preventive dentistry today.
Further information can be found at the German Society of Periodontology (DG PARO) and the Federal Dental Association (BZÄK). If there are signs of periodontitis, we recommend our specialised periodontology treatment.
Costs of PZR and Health Insurance Contributions
The costs for a professional dental cleaning in Munich vary depending on the scope of treatment, the method used and the duration of treatment. We place the greatest value on transparency: before we begin, you know the price.
Why do prices vary? The decisive factors are: the number of teeth to be cleaned, the degree of deposits (little or a lot of tartar?), the method used (standard PZR, Air-Flow or GBT), and whether implants need to be cleaned. A standard PZR with good oral hygiene takes approx. 45 minutes; with heavy deposits or GBT, treatment can take 75 minutes.
What do statutory health insurers pay?
Statutory health insurance does not cover the PZR as a standard benefit — it is an individual health service (IGeL). However, many insurers subsidise the PZR through their bonus programmes or statutory services. Here is an overview of common subsidies (as of 2025/2026 — figures without guarantee, please confirm with your insurer):
| Health Insurer | PZR Subsidy | Frequency |
|---|---|---|
| Techniker Krankenkasse (TK) | Up to 40 EUR | Once per year |
| AOK Bayern | Up to 50 EUR | Once per year (bonus programme) |
| Barmer | Up to 50 EUR | Twice per year |
| DAK-Gesundheit | Up to 60 EUR | Twice per year |
| IKK Classic | Up to 40 EUR | Once per year |
Figures without guarantee — please confirm the exact subsidy directly with your health insurer.
Private health insurance: Most private health insurers cover the PZR fully or to a large extent — depending on the tariff and preventive bonus. We will issue you a detailed invoice according to the GOZ (German Dental Fee Schedule) after treatment, which you can submit to your insurer.
You can also use our fixed subsidy calculator to calculate your individual reimbursement entitlements.

Before & After — What to Expect Following Your PZR
Many patients are surprised by how noticeable the difference is after a professional dental cleaning — both visually and in terms of how it feels. Here we describe realistically what you will experience immediately after treatment and in the following weeks.
Immediately after the PZR: Your teeth feel smooth and fresh — a feeling you only truly appreciate once you have experienced it. Staining from coffee, tea or red wine is significantly reduced or completely gone. The gums may remain slightly sensitive for a short time, which is completely normal and resolves within 24 to 48 hours. The fluoride application leaves a slightly chalky film on the teeth — this is intentional and shows that the fluoride varnish is working.
In the first few days: The gums, which were previously slightly swollen due to inflammation, recede and appear more defined. In some patients, the teeth appear slightly lighter after the PZR — this is not due to bleaching, but because the natural tooth colour comes back into view once discolourations have been removed. For actual tooth whitening, we recommend our professional bleaching.
Long-term: With regular PZR, the gums remain healthier, cavities occur less frequently, and tartar builds up more slowly. Patients who consistently attend for prophylaxis have measurably shallower gum pockets and significantly fewer tooth losses with age. That is the true promise of the PZR: not just beautiful teeth today, but healthy teeth for decades.
