How long does aligner treatment take?
The duration of aligner treatment depends on the complexity of your misalignment. In normal everyday situations, during conversations, at meetings, and in photos, aligners are practically undetectable. Many of our patients report that friends and colleagues only noticed their ongoing treatment months later. The difference compared to visible brackets and metal wires in terms of discretion is enormous.
Does health insurance cover the cost of aligners?
For adults, statutory health insurance (GKV) generally does not cover the cost of aligner treatment. For children and adolescents with an orthodontic indication rating (KIG) of 3–5, a statutory contribution may apply. Private health insurance and supplemental dental insurance policies with an orthodontic add-on reimburse 50–100% of treatment costs depending on the tariff. How many hours a day do I need to wear aligners?
The medically required minimum wear time is 22 hours per day — leaving you 2 hours daily for meals, brushing, and cleaning the aligners. Consistently falling below this wear time risks an incomplete treatment result and may require additional refinement aligners. Many patients wear their aligners overnight and remove them only to eat and brush — making the wear time easy to maintain. The 22-hour rule is not a recommendation — it is a medical necessity.
Does aligner treatment hurt?
Aligners do not cause actual pain, but many patients describe a pressure sensation or mild sensitivity in the first 1–3 days after each tray change. This is a good sign — it means the teeth are moving. The pressure sensation diminishes with each new aligner, as tooth movements per step are controlled and gentle. Can I eat and drink normally with aligners?
Yes — for eating and drinking, simply remove the aligners. This is one of the greatest advantages: no dietary restrictions, no forbidden foods. When it comes to drinks: plain water can be drunk with aligners in, but coffee, tea, juices, and soft drinks should only be consumed without aligners — staining and sugar trapped under the tray are undesirable.

With Invisalign, longer intervals may be possible for uncomplicated cases. Compared to fixed braces, check-up appointments are shorter and less frequent — a further advantage of aligner therapy.
What happens after aligner treatment? (Retention)
Retention after the active treatment phase is just as important as the treatment itself. Without retainers, teeth drift back to their original positions — a biologically inevitable process. We generally recommend a fixed wire retainer (bonded to the inside of the front teeth) for permanent stabilisation, as well as a removable Essix retainer for the first 12 months. After that, wearing the retainer at night is sufficient. The retainers protect the result you have invested time and money in.
Are aligners better than fixed braces?
Neither aligners nor fixed braces are universally "better" — they have different strengths and indications. Aligners are more discreet, more comfortable, and oral hygiene is simpler. For large gaps or missing teeth, aligners can be a first step to optimally prepare space for implants or other restorations such as ceramic inlays or veneers.
